Case Managers shall be responsible for the implementation of Individual Recovery and/or Treatment Plan at the direction of the Case manager Supervisor for Intensive Case Management (ICM) to help each client achieve optimal stability and functioning.
CM's shall be familiar with priority client cases, execute tasks in a timely manner, develop resources and establish a “community contacts” data base for immediate access of resources.
CM's shall be familiar with the functions of all FAN, LLC therapeutic guidelines,and, how the CM position can best implement these guidelines to serve the needs of the client.
CM will implement agency operations, policies and procedure and will ensure that the lines of communication between Administration, the field (therapists, and family members, community providers) and the client remain open.
Minimum Level of Knowledge, Skills and Abilities Required to Perform the Position Responsibilities:
A. Skills in writing, spelling, word processing, data entry and e-mail communication.
B. Ability to perform basic mathematic computations
C. Ability to effectively communicate with all FAN, LLC clients via home visits, telephone or in the office.
D. Ability to communicate with and coordinate services between FAN, LLC office support staff, Therapists, Case Managers, Mental Health and PSR Specialists, Clients, Caregivers and Managed Care Companies.

Required Education/Experience:
1 year of Case Management and/or Direct Mental Health Care experience with Target Population AND/OR:
• Bachelor's Degree in Social/Behavioral Science Field (QMHP)
• Master's Degree in Social/Behavioral Science Field and Licensure (QMHP-CS and LPHA)
• Associate's Degree and 2 years experience (CSSP)
