POSITION SUMMARY:
The Toronto Centre for Liver Disease located at Toronto General Hospital is a University Health Network Centre of Excellence for viral hepatitis, autoimmune liver disease and liver cancer. Part of the mandate of the centre is to oversee and coordinate an Ontario Ministry of Health program called the Ontario Roadmap for Hepatitis C Elimination that is a guiding strategy to achieve elimination of hepatitis C virus infection as a public health threat in Ontario. The current role will be focused on supporting implementation of the Roadmap vision.
Working under the direction of the leadership team, the Project Manager utilizes knowledge of project management and exercises judgment and decision-making skills to advance the tools for implementation and monitoring progress of a coordinated provincial response to eliminate hepatitis C in Ontario. Working closely with team members and partners from multiple sectors, this position is responsible for leading strategic partnerships and collaborations, including with provincial health care partners, government, community agencies and researchers. This position is responsible for providing leadership to the core project team and is accountable to the Ontario Hepatitis C Elimination Roadmap Steering Committee Leadership Team. This position will work closely with a provincial counterpart based at the CATIE, Lead, Hepatitis C Elimination.
DUTIES:
• Convene and support population and sector leaders to plan and implement Roadmap “early wins” and develop and execute multi-year plans to increase access to information, testing and treatment
• Develop and implement a process to define targets and baselines, and oversee process on available data
• Establish and support a Data Collaborative, a group responsible for addressing data gaps, coordinating data sharing, ongoing monitoring and routine reporting
• Support development of and linkage to clinical tools and supports, including ongoing engagement with clinical providers and organizations (including professional associations)
• Develop and implement a plan for provider tools, including a needs assessment to identify existing tools and gaps and strategies for engaging stakeholders
• Ongoing work with partners to develop and promote tools and resources
• Planning and implementation of educational and capacity building efforts for clinical and community providers
• Continued support to connect providers and organizations to trainings, and additional support to help set up hepatitis C programs (including QI initiatives)
• Work with key stakeholders to define requirements for a “one stop shop” website (including a dashboard for providers) and establish an end-to-end plan for implemention
• Transition of Roadmap website to elimination “one stop shop”, with ongoing maintenance
• Develop and implement a mechanism for community partners to access training and order point-of care antibody and dried blood spot testing, including quality assurance oversight
• Identify what is needed for continued sustainability of a coordinated plan to reach elimination targets
QUALIFICATIONS:
• Master’s Degree and Bachelor’s degree in Clinical/Medical/Biologic Science, Business or Health Adminstration, Health Informatics, Epidemiology or related field required
• At least five (5) years practical and related experience required
• Project Management Professional (PMP) designation preferred
• Project planning, management and implementation experience required
• Demonstrated ability/experience leading large scale, multi-year and multi-stream projects required
• Experience working with health administrative data strongly preferred
• Knowledge and understanding of Ontario’s health care system, health data and evaluation frameworks
• Background in viral hepatitis C, community outreach and public health an asset
• Excellent interpersonal skills, and demonstrated ability to take initiative and work effectively, both independently and collaboratively
• Proven track record working with and managing relationships with a variety of stakeholders to achieve aligned goals, including researchers, clinicians, community, policy and system leaders
• An understanding of health equity, infectious diseases and population health
• Excellent problem-solving, organizational and time management skills
• Excellent verbal and written communication skills in English
• Ability to effectively handle confidential matters and materials
• Ability to work well under pressure
• Computer proficiency (Microsoft Office environment, Word, PowerPoint and Excel)
